Admissions Information
The participant is at least 18 years of age.
The participant’s Individual Program Plan (IPP) includes objective(s) that can be addressed through adult services.
The participant’s Individual Program Plan (IPP) includes objectives that can be achieved through the services we provide.
Admissions Procedure
Case Manager , Care Provider, or Family inquires with Program Director
Case Manager submits Information packet (See Complete Packet instruction below)
The Program Director will reach out to the the participant, family, care provider, and case manager.
The Program, ID Team, and consultants then determine whether the participant is an appropriate fit for the services offered by BCS.
A Complete Information Packet includes
Current IPP
Current Medical Info (i.e., diabetic, seizure disorder, heart problems, etc)
Current CDER
Current or previous day program’s ISP,IEP, Behavior Plan
Current Annual Report and Behavior Plan
